ADMINISTRATOR

Responsibilities:

Carry out the policies of the Adult Day Health Care center according to regulations and established policy, program and budget. Develop an administrative plan and procedure to insure clearly defined lines of responsibility, equitable workloads and adequate supervision

Minimum Qualifications:

  • Knowledge of supervision and care appropriate to the participants receiving services;
  • Maintain and supervise the financial and other records;
  • Masters degree plus one year of experience in a responsible managerial, administrative, or supervisory position in a social or health service program or agency OR a bachelors degree and three years of experience as above;

PROGRAM DIRECTOR

Responsibilities:

The Program Director shall coordinate, implement and supervise all programs and services of Grace Adult Day Health Care Center. The Program Director shall be at the center during all times of center operation and will act as the Administrator during periods of the Administrator’s absence. All policies and procedures of Grace Adult Day Health Care center, applicable laws and licensing requirements will be followed at all times.Minimum Qualifications:

  • Qualified professional in the field of nursing, social work, psychology, or recreational, occupational or physical therapy with demonstrated or potential competence in working with impaired, elderly living at home.
  • Shall be knowledgeable of physical, social and mental health programs operating within a licensed health facility or clinic.
  • Educational requirements depend upon the discipline of focus and must meet state requirements. Three years of demonstrated experienced in the methods of administration, management and organization; program development; personnel management.

REGISTERED NURSE and LVN

Responsibilities:

Assess each participant to determine needs for personal care and assistance in activities of daily living. Provide health education and counseling to the participants and monitor health status. Track and record participant’s health improvements.

Minimum Qualifications:
  • Completed nursing school with a current license to practice, preferably as Public Health Nurse licensed by California Board of registered Nursing.
  • Knowledge of public health with reference to aging, physical assessment and care planning, health counseling, and community health services and facilities.
  • Demonstrated skills and experience to assess and monitor elderly health care and conduct health education/training programs.

SR. SOCIAL WORKERS

Responsibilities:

Provide all social work services for Grace Adult Day Health Care Center. All policies and procedures of Grace adult Day Health Care center, applicable laws and licensing requirements will be followed at all times.

Minimum qualifications:

  • A master degree in social work from an accredited school of social work is required, with three years of recent case management experience with two years geriatric experience required. Other graduate degree may be considered with approval from the state of CA.
  • Must have oral and written skills in order to prepare and present complete and concise written materials and make presentations to community groups.
  • Effective time management skills. Prefer bilingual (FARSI).

SOCIAL WORKERS

Responsibilities:

Under guidance from the Program Director, provide in-home and social services assessments for all Adult Day Health Care participants. Meet with family members or referring agency and determine appropriate programs. Provide group sessions and family counseling as needed. Seek guidance from and consult with staff members regarding the relationship of social, emotional and ethnic factors to health and medical care. Provide discharge planning for participants, including assessment and recommendations for financial assistance. Provide signed and dated documentation for all services performed as well as quarterly progress notes and six-month reassessments on participants to management.

Minimum qualifications:

  • Master's degree in Social Work and three months experience in adult day health program.

PROGRAM AIDE

Responsibilities:

Provides supportive services to the participants under direction and supervision of a professional staff member, as assigned by the Program Director.

Minimum Qualifications:
  • High school graduate or equivalent. The ability to comprehend both written and spoken English and Farsi.
  • An interest and desire to work with the Persian population served by the center.
