Mission
Grace ADHC provides medical, rehabilitive and social services to elderly persons and other adults with functional impairment, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self care. Grace ADHC will provide top quality services through an individual plan of care and targets rapid growing number of adults who are at the institutional level of care or at risk of institutional placement. Grace ADHC is a Medi-Cal benefit program and is administered by California Department of Aging (CDA) and licensed by the State Department of Health Services as a health care facility.

Here are the some of the services that will be provided by the professional staff at Grace ADHC Center:

Medical reviews

A Nurse
who observes and monitors the person's health status and medications and who assists with any treatments needed.

Physical and Occupational Therapists
who design a specialized therapy program to improve and maximize strength, endurance, range of motion and self care skills. The program is carried out at the center.

Nutritional services
that include dietary evaluation, instruction, snacks & a hot lunch.

Social Service
assistance for group and individual counseling, advocacy, information and referral that address any questions or additional service needs someone may have.

Social and Recreational Activities
on an individual and group basis that increase the opportunity for socialization and decrease isolation.

Speech Therapy
services that evaluate for speech and language disabilities and provide any instruction or therapy needed.

Transportation
to and from home at no extra charge
